Gym Liability Waiver

Florida Coastal Prep — Gym Liability Waiver & Release
In consideration of being permitted to use the facilities, equipment, and programs of Florida Coastal Prep ("FCP"), I hereby agree to the following:
1. Assumption of Risk. I acknowledge that physical exercise and use of gym equipment carries inherent risks, including but not limited to physical injury, muscle strains, cardiovascular events, and other health complications. I voluntarily assume all such risks associated with my participation and use of FCP facilities.
2. Release of Liability. I, on behalf of myself, my heirs, personal representatives, and assigns, hereby release, waive, and discharge Florida Coastal Prep, its owners, officers, coaches, employees, agents, and volunteers from any and all claims, demands, losses, damages, and causes of action arising from my use of FCP gym facilities and equipment, whether caused by the negligence of FCP or otherwise.
3. Medical Fitness. I represent that I am in good physical health and have no medical conditions that would prevent safe participation in physical exercise. I agree to consult a physician before beginning any exercise program if I have any doubts about my fitness level.
4. Rules and Conduct. I agree to follow all FCP facility rules and staff instructions. I understand that failure to follow rules may result in removal from the facility. I accept responsibility for any damage I cause to FCP equipment or property.
5. Emergency Treatment. I consent to emergency medical treatment in the event that I am incapacitated and unable to make decisions. I understand that FCP is not responsible for any costs related to such treatment.
6. Governing Law. This agreement shall be governed by the laws of the State of Florida. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.
By signing below, I confirm that I have read, understand, and agree to the terms of this waiver. I acknowledge that this is a binding legal agreement and I am signing it freely and voluntarily.
